Planning a Visit?
Dr. House makes a commitment to serve each patient’s dental care needs with the time that quality care requires. Regular six month dental check ups and cleanings are one hour appointments for everyone except small children who require shorter visits. Regular check ups ensure that your teeth are kept healthy and that any problems can be addressed promptly before they become more extensive and more expensive to treat.
Based on the time required for procedures that need to be performed, Dr. House schedules restorative and cosmetic dental treatments by appointment following one’s regular check ups and examinations. Emergencies can occur at any time, and every effort will be made to see our patients of record as soon as possible when emergencies arise.

Because both your time and that of Dr. House and his staff is important, 24 hour notice of cancellation of appointments is required.
What Do I Need For My Initial Visit?
We will need a copy of your driver’s license or government issued ID, a copy of your insurance card or dental benefit plan information, your payment (we accept debit or credit card, money orders, or cash for the first visit), and the following forms you can fill out prior to your arrival: patient information, medical dental history.
Insurance
As a courtesy to our patients, we file with your dental plan. Deductibles, co-pays, and the portion your dental plan does not cover are due at the time services are provided. Most dental plans pay on claims within 30 to 45 days.
Payment
We accept cash, checks, debit cards, and most credit cards for payment. A prompt payment courtesy of 5% is offered for full payments made by cash, check, and debit card at the time services are provided for those without a dental care plan.
Billing
We offer Care Credit for our patients who want low or no interest financing for dental care needs. Our office staff can assist you with any questions you have at (318) 949-9879.
